Geriatric Care Management (GCM)
Trained professionals who can help navigate the Senior Care Maze. From providing information about Medicare Guideline Rights and Care Plan Development, to finding a doctor, a GCM will help you navigate the choices one must make to set a quality plan in place. ($0 - $150 per hour Private Pay)
Placement Services
Our teams does a Comprehensive Intake, including a medical history, and, we do a visual assessment of the client. We do, Care Facility Research and set up Tours, guiding your family to the BEST options, fitting their level of care needs, budget, and location criteria. (This service is FREE for our clients)
Support Groups
On the last Sunday of each month we meet at a different winery, brewery, or coffee shop to provide our clients and other Primary Caregivers of Seniors with support, guidance, and friendship at our "Wine about your Aging Parents, BREW UP a Better Care Plan" support group. (Only cost is what you wish to eat or drink.)
VA Screening
We Provide our clients who are Veterans, or spouses of Veterans, or widowed spouses, with VA benefit screening assistance. (Cost is FREE to our placement clients.)
Educational Workshops
We offer educational "Aging in Place" and "Care Plan Development" workshops. Contact us for more detail and to schedule one for your senior group, church, club, or community. (Cost is FREE to groups with 20+ attendees)
General Senior Care Options
This is a general breakdown of Senior Care Options in the community: (We provide referrals to quality sources within each of the categories listed below.)
Independent Living (ILF)
ILF’s are an ideal apartment setting for seniors wishing to downsize, enjoying the benefits of house keeping, some meals, social events, transportation to stores and doctor appointments, and much more. They are more expensive than in a regular senior community because they offer things those communities don’t. They do not offer assistance with Activities of Daily Living (ADL’s). ($2,000 - $4,000 Private Pay)
Non-Medical In Home Care
This can be anything from companion care, to total care, or end of life comfort care, for seniors living at home, needing assistance with Activities of Daily Living. Private duty care givers come to your residence, to assist with varying levels of ADL's, housekeeping, transportation, socialization, meal preparation, and much more. ($21 - $29 per hour Private Pay)
Respite Care
This service allows the caregiver of a senior to refresh & restore their energy. By handing over the care responsibilities of the senior, this gives the at-risk senior an opportunity for socialization in a safe setting, while allowing rest for the primary caregiver. This can be done in the seniors home, in a skilled facility, or in assisted living communities. (Prices vary depending upon where you receive services. Private Pay, Long Term Care Polices, Medi-Cal/Medi-Caid)
Assisted Living (ALF)
These are Non-Medical, Residential Care Facilities for the Elderly or “RCFE's", licensed through, California's DSS, under Title 22. They are large apartment style communities, offering 24/7 monitoring, social events, and are ideal for seniors who are active and need only minimal help with ADL's, but are just tired or unable to clean, prepare meals, or drive. *(Cost is Private Pay monthly fee + one time community fee - $4000 - $10,000 per month)
Board and Care
These are Non-Medical, Residential Care Facilities for the Elderly or “RCFE's", licensed through, California's DSS, under Title 22. They are community-based homes with 6 or fewer people, They offer 24/7 hands on care, for seniors needing higher levels of care and assistance with most ADL’s. (Private Pay, Long Term Care Polices - $2000 - $6000 per month)
Memory Care
These RCFE options are secure and provide caregivers specially trained in behaviors related to the dementia process, experienced by many suffering with some form of Dementia/Alzheimer's. They offer a special setting where every detail is focused on activities and an environment meant to comfort the resident and help them thrive. (Cost is Private Pay/Longterm Care Policies - $3000 - $7,000 per month + one time community fee)
Home Health
These are teams of certified "Medical Professionals" who provide ‘short-term’ Physical Therapy, Occupational Therapy, or other medical needs, for someone recuperating after an Acute or Sub-Acute hospital stay, and to obtain services, a prescription from a physician is needed. (Medicare, Medi-cal/Medicaid when qualified, no cost)
Skilled Nursing Facility (SNF)
These are Sub-Acute hospitals, where seniors with “qualified stays” in Acute Care hospitals, recuperate and do intensive rehabilitation, in preparation to return to their home, be it an ALF, ILF, B&C, or Long Term Custodial SNF environment. (65+ Short-term paid for by Medicare/HMO/PPO/ Medicare Supplements - usually $0 Cost) (65- Insurance or Medi-cal/Medicaid paid - $0 Cost depending on insurance type)
Hospice
This service comes to you. Dr.’s, Nurses and CHHA’s, in a conjunction with a qualified care plan, provide end of life comfort care, when curative measures are no longer working. Medicare, recipients do not pay for this as it is a free service. (Medi-cal/Medicaid./private insurance - $0 Cost)
